Qualifications:
One or more of the following criteria:
One year minimum of continuous experience as a Home Health Aide or Nurse
Aide in a health care environment; Completion of a State (or similarly
accredited) Home Health Aide or Nurse Aide training program; Completion
of one year of classroom and clinical training from an accredited nursing
program, medical assistance program, or other applicable direct care training
program.
Satisfactory compliance with ComForcare
bonding criteria.
Demonstrated ability to effectively
communicate and interact with clients, families, co-workers, and all supervisors
in the English language .
Ability to accurately follow and process
written and verbal instructions.
Satisfactorily demonstrate and perform
competency in clinical skills through competency checklist and skills demonstration

Job description
The HHA/CNA provides specifically
defined, non-medical care services under the supervision of a Registered
Nurse to enable the client to remain in the home. The HHA/CNA is a non-licensed
health care employee who provides personal care and homemaking services
in the home setting to clients and families who are unable to perform these
activities independently. The HHA/CNA conforms to all agency policies and
procedures including providing care, compliance with the agency Bill of
Rights, observing client/family confidentiality, and adhering to agency
dress code.
Routine duties - personal care services
Bathing, assistance with dressing,
nail and skin care, back care, foot care, shampoo and oral hygiene.
Assistance with ambulation, transfers,
range of motion exercise, safe use of equipment and assistive devices,
(walker, crutches, cane) change of position.
Assistance with toileting; use of
bedpan, commode, or urinal; incontinent care.
Meal planning, preparation, and assistance
with feeding.
Emotional support and motivation through
conversation, encouragement and recreational activities.
Maintenance of appropriate agency
records.
Present and conduct themselves in
a professional manner according to agency policy.
Adhere to infection control and safety
procedures which are in accordance with agency policy.
Household management
Light Housekeeping- including changing
of bed linens, vacuuming, sweeping, dusting of client's environment, emptying
client's wastebaskets and garbage. Refrigerators are defrosted, when necessary,
if client or caretaker is unable to do so. Cleaning of the bathroom includes
bathtub, sink, and toilet. The floor should be vacuumed and mopped if it
becomes wet. Cleaning of kitchen includes washing dishes, counter tops
and the floor.
Personal laundry- That laundry that
is essential to client's health care. Laundry for any family members or
others in the same home is not the responsibility of caregiver.
Grocery shopping.
Services that are not permitted by
companions, but are permitted by HHA/CNAs
Catheter Care- Emptying catheter bags
and measuring intake/output.
Monitor/Record vital signs.
Provide care for any assistive devices/equipment
(braces, slings, walker, and/or hoses).
Monitor intake and/or output.
Provide diabetic care, including monitoring
hypoglycemia or hyperglycemia, urine testing, and preparation of special
diet.
Provide incontinent care- changing
briefs, etc...
Companions can provide assistance
in basic patient transfers as long as the patient has been assessed as
capable of ambulating without assistance and/or another trained caregiver
(including family) is involved .
Application of warm/cold compresses
for any clinical symptoms. A companion can prepare a compress under patient,
nurse, or administrator direction in an emergency situation.
Assistance with Range of Motion exercises.
Exceptions can only be made at discretion
of Director of Operations and RN/SW assigned to case.
Home Companion (position description)
Position: Home Companion
Supervised by: Administrative Coordinator or Director of Operations
Minimum qualifications:
Current Driver's License, Proof of
automobile insurance, and daily availability of a car for business use.
Satisfactory compliance with ComForcare
bonding criteria.
Demonstrated ability to effectively
communicate and interact with clients, families, co-workers, and all supervisors.
Including ability to proficiently follow and process written and verbal
instruction in the English language.
Satisfactorily demonstrate and perform
competency in direct care skills through completion of competency checklist.
Desired qualification:
Six months of continuous direct care/home
care experience.
Physical requirements
Eighteen years of age or older.

Position: Social Work Care Manager (SW)

Minimum qualifications:
Current State Social Work license.
One year of continuous social work
experience or equivalent.

Concise verbal, written, and organizational
skills.
Desired qualifications:
Extensive assessment skills.
Experience working with JCAHO standards.

Job description:
The SW is responsible for assessing
and reassessing the needs- including the cognitive, psychosocial, and environmental-
of clients and their families. The SW is not responsible for assessing
hi-tech personal care needs of clients. The SW develops an appropriate
and individual care plan, collaborating with other health care providers
as needed, and is responsible for teaching/counseling clients regarding
self-care and evaluating client response to care provided. Acting as the
care coordinator for assigned individuals/ families, the SW initiates referrals
as necessary, supervises and directs Home Companions, Home Health Aides,
families, and other caregivers regarding care of the client.
General responsibilities:
Adhere to agency infection control
and safety procedures by performing and documenting observations, procedures,
and interventions in the home according to the established care plan in
a safe and competent manner.
Effectively communicate and interact
with clients/caregivers and families in a supportive manner.
Management of client caseload and
associated or other delegated work responsibilities in an effective manner
within the resources of the agency and according to agency standards.
Interpretation of ComForcare programs
and policies as appropriate to clients and members of the community.
Participates in the continuing commitment
for quality care, including attendance at mandatory agency staff meetings.
Assessment responsibilities:
Initiation of the Care Plan including
the securing and updating of complete and accurate health information on

accountant, and file flow sheets in client files.

Routine Duties:
Coordinator's primary responsibilities
involve daily coordination of client schedules. The coordinator is responsible
for staffing clients with agency-deemed suitable employees and assuring
maximum employee attendance ratios. Staffing responsibilities include emergency
staffing, coordinating monthly availability of field staff, responding
to customer requests in an efficient and professional manner, reporting
customer/employee concerns to relevant supervisory staff, briefing new
employees to client care plans for initial visits, and initiating disciplinary
action for employees.
Payroll coordination- Coordinating
employee flow sheets with client schedules, data entry of billing and payroll
data into software system, follow-up with field staff who are delinquent
with paperwork..
Employee recruitment. ComForcare Senior

Various clerical duties, at the discretion

The Administrative Coordinator is
responsible for after-hours on-call scheduling duties during alternating
weeknight and weekend hours. These duties include fielding urgent client
and employee calls, coordinating staffing substitutions, and coordinating
new client intakes. On-call duties are restricted to telephone coordination
only (no off-site travel), and are initiated only after complete familiarity
with scheduling position.

Ultimate goal of position is controlled
and sustained company growth while maintaining and assuring high standards
of service. This includes responding to customer requests in an efficient
and professional manner, while reporting customer/employee comments and
concerns to relevant supervisory staff.
Sales efforts will be focused primarily
on travel in the franchise territory, initial training time by the Director
of Operations will be required.
Sales goals and objectives will be
planned and discussed prior to significant efforts being expended. ComForcare
management will provide input as to how and where to most efficiently allocate
sales efforts. This position will provide significant autonomy within the
broad constraints of predetermined goals and objectives. Frequent planning
and debriefing meetings will be required to interpret and communicate sales
progress, including regular staff meetings to update all administrative

be utilized to help correlate sales efforts with results and to meet IRS
expense reporting purposes.
